Use this form to report an item lost on UTA vehicles or property.



After completing the form, you will receive a reference number and email confirmation. The description you provide will be kept on file and compared to found items for up to 30 days. Please describe the item in detail and provide a photo, if possible, to increase the likelihood of retrieving your item.

Ski Bus

If you lost an item on a UTA Ski Bus (972, 994, 677, 674, 675), please fill out the form above or call UTA Customer service at 801-RIDE-UTA (801-743-3882).

You will be notified as soon as there is a match to your report. Found items are stored for 30 days, after which unclaimed items are donated.

UTA On Demand

If you lost an item on UTA On Demand, email support-SLC@ridewithvia.com or call 385-217-8191.

CS1 and CS2
